What does Achlorhydria mean?

Acidifying agents

Acidifying agents are used in condition where there is absence or insufficient secretion of hydrochloric acid in stomach which leads to disease called Achlorhydria or Hypochlorhydria. Achlorhydria can be treated by various acidifying agents like ammonium chloride, dilute HCl, Calcium chloride etc.

Causes of Achlorhydria:

What causes Achlorhydria?

The cause of achlorhydria in first case may be subtotal gastrectomy, atrophic gastritis, carcinoma, gastric polyp etc while in later case it may be chronic nephritis, tuberculosis, hyperthyroidism, chronic alcoholism, sprue, pellagra etc. The symptoms vary with associated disease but they generally include mild diarrhoea or frequent bowl movement, epigastric pain and sensitivity to spicy food.

The pH of stomach is 1.5 -2 when empty and rises to pH 5-6 when food is ingested. The pH of stomach is so low because of the secretion of HCl. Gastric HCl act by destroying the bacteria in the ingested food and drinks. It softens the fibrous food and promotes the formation of the proteolytic enzyme pepsin. This enzyme is formed from pepsinogen at acidic pH (>6). Pepsin helps in the metabolism of proteins in the ingested food. Therefore lack of HCl in the stomach can cause Achlorhydria.

  • Pernicious anemia (anti-parietal and anti-intrinsic factor antibodies): It is an autoimmune phenomenon in which antibodies are formed. These autoantibodies cause the autoimmune destruction of parietal cells leading to atrophic gastritis.
  • Use of antisecretory medications: Short-term standard dose treatment with PPI has been shown to have low risk, but long-term use of PPI has been linked to hypochlorhydria.
  • Helicobacter pylori infection: Acute H. pylori infection of gastric epithelial cells represses H-K-ATPase alpha subunit gene expression leading to transient hypochlorhydria and supporting H. pylori proliferation. This growth of H. pylori may even initiate a pathological process causing gastric cancer.
  • Gastric bypass: Gastric bypass is a gastric exclusion operation performed in patients with massive obesity to reduce food intake. Studies show that the acid secretion unbuffered by the food in the excluded stomach results in lowered gastrin secretion hence leading to achlorhydria in such patients.
  • VIPomas: VIPoma is an endocrine tumor that usually arises from beta-pancreatic cells and secretes Vasoactive intestinal peptide. It may be associated with MEN1. The massive amounts of VIP secretion cause watery diarrhea, hypokalemia, achlorhydria, vasodilation, hypercalcemia, and hyperglycemia.
  • Hypothyroidism: Thyroid hormone plays a role in hydrochloric acid secretion hence hypothyroidism can lead to achlorhydria.
  • Radiation to stomach: Radiation to stomach has also been reported to cause achlorhydria.

Types of achlorhydria

Two types of achlorhydria are known:
1) where the gastric secretion is devoid of HCl, even after stimulation with histamine phosphate

Narcotic drugs and psychotropic substances are the habit forming substances which may produce a sleep. At higher concentration, those substances proved to be toxic and therefore, there is a restriction for the cultivation, collection, import, manufacture, purchase, sale, transportation of narcotic drugs and psychotropic substances. Psychotropic substances are substance, salt or any preparation which is included in the list of narcotic substance as specified in the Schedules to the Act.

Cannabis

Charas :

separated resin whether crude or purified obtained from Cannabis plant and also includes concentrated preparations and resin known as hashish oil or liquid hashish.

Ganja:

Flowering and fruiting tops of Cannabis plant excluding from seeds and leaves, or

Any mixture with or without neutral material of any above forms of Cannabis or any drink prepared from them.

Coca derivatives:

Crude cocaine which can be used directly or indirectly for the manufacture of cocaine; or

  1. b) Ecgonine and all the derivatives of ecgonine from which it can be recovered; or

What is Pharmacy ACT about?

The PharmacyAct, 1948. The need of the qualified and trained pharmacists and their registration to respective Councils was re-emphasized by the Health and Development Committee, 1945 constituted under the Chairmanship of Justice Bhore. The foundations were laid for The Pharmacy Act based on the recommendations of Drugs Enquiry Committee and Health Survey and Development Committee. The Pharmacy Act, 1948, came into existence with the objective to regulate the profession and business of pharmacy.

The uncontrolled and unrestricted advertisements related to drugs and medicines were freely published making obscene, false and exaggerated claims for their medicines. Hence, Drugs and Magic Remedy (Objectionable Advertisements) Act, 1954 came into existence with the objective to control and prohibit such advertisements of drugs which makes false claim for the drug or which gives the misleading information about the drug.

Medicinal and Toilet PreparationsAct, 1955
Alcohol is the important solvent in pharmaceutical industry. Each State had different sets of excise rules and different rates of duties on alcohol. To provide for the levy and uniform collection of duties of excise on the medicinal and toilet preparations, Medicinal and Toilet PreparationsAct, 1955 came into existence.

Timeline of Pharmacy Regulations 

1930: Government of India on 11th August 1930 , appointed a committee under the chairmanship of Late Col. R.N.Chopra to see into the problems of Pharmacy in India and recommend the measures to be taken.

1931: R.N. Chopra committee published its report in 1931. It was reported that a set of people known as compounders were filling the gap.

1932: Just after the publication of the report Prof. M. L. Schroff (Prof. Mahadeva Lal Schroff) initiated pharmaceutical education at the university level in the Banaras Hindu University. The Department of Pharmaceutical Engineering and Technology (Formerly Department of Pharmaceutics) was established in 1932 in BHU.

1935: In 1935 United Province Pharmaceutical Association was established which later converted into Indian Pharmaceutical Association.

1939: The Indian Journal of Pharmacy was started by Prof. M.L. Schroff in 1939.

1940: All India Pharmaceutical Congress Association was established in 1940.

1940: Drug and cosmetics Act 1940 and Rules 1945

The primary objective of the Act is to ensure that the drugs and cosmetics sold in India are safe, effective and conform to prescribed quality standards. The Drugs Act was formulated in 1940 in pursuance of recommendations of Chopra Committee constituted in 1930 by Government of India.

1940: Central Drugs Laboratory was established in Calcutta

1941: The first Drugs Technical Advisory Board (D.T.A.B.) under this Drugs act 1940 was constituted.

1948: Pharmacy Act 1948

An Act to regulate the education and profession of pharmacy. Constitution of national pharmacy council and state pharmacy councils for the aforesaid purpose.

1948: Indian Pharmacopoeial Committee was constituted under the chairmanship of late Dr. B.N. Ghosh.

1949: Pharmacy Council of India (P.C.I.) was established under Pharmacy Act 1948.

1954: The Drugs and Magic Remedies (Objectionable Advertisements) Act, 1954.

Telangana State Pharmacy Council is a statutory body constituted by the Government of Telangana State under the provisions of the Pharmacy Act of 1948. The main objective of the council is to regulate the profession and practice of pharmacy in the state of Telangana.

The prime function of the TSPC is to grant registration to the eligible pharmacists possessing requisite qualification as per the provisions of section 32(2) of the Pharmacy Act and to enforce the necessary provisions of the Pharmacy Act 1948 and Telangana State Pharmacy Council Rules, 1955.

B Pharmacy registration procedure:

1. Must be a Resident or Practicing in the Telangana State
2. If Employed must disclose Designation, Institution name and place of working
3. Must have valid Email ID and Mobile Number
4. Book a Slot date Online. Personally attend the Council office along with Originals for Endorsement, HardCopies of uploaded Documents and also other related Certificates for verification on the Slot Date
5. Fill Date Of Birth exactly as in S.S.C / S.S.L.C / Matriculation Certificate
